Purpose:

The immunization clinic offers various types of immunizations.

Remarks:

The Rock Falls office offers immunizations. Both walk-in and appointment times available Monday- Friday 8:30-4 pm. Please call for more information. Previous immunization records need to be available. Parents or legal guardians must sign a consent form for children. There are no income guidelines.
