Purpose:

Designed to help qualifying women and children get off to a healthy start and to stay healthy. Family Case Management helps pregnant women in obtaining essential prenatal care, provides developmental screening for children and assistance to families with infants in obtaining appropriate well child examinations, medical screenings and immunizations.

Remarks:

Eligibility: Pregnant women, infants and children who are eligible for WIC qualify and are urged to get involved in this program.
