New leadership: This Skilled Nursing Facility (SNF) is owned and operated by Stephenson County, but now managed by Transitional Care Management. We have a new Administrator, new clinical programming and we are expanding our team.

With 148 beds, we serve a diverse population of short-term rehab patients, long term care residents and memory care. We have an active therapy dept. achieving terrific functional gains and a remarkable restorative program.

Job Description

What you will do in this role: In our facility we feel the CNA & Nurses are pivotal to the satisfaction of a patient/”Guest”. You make a direct impact on a patient’s recovery and stay. You will help patients reach their goals and work side by side with a highly skilled multidisciplinary team.

Our team members are awesome; they do whatever is needed to make the patient more comfortable.
